What Home Healthcare Looks Like.

We can arrange scheduled visits by our trained medical professionals to carry out check-ups at the convenience of your loved ones in their homes. Our healthcare professionals will record standard vitals such as blood pressure, weight, blood sugar levels, etc. We can also check on medication to see if prescriptions need to be collected or if their running low. Align Care Plus will report all the information to the sponsor and necessary medical bodies.

Our different service packages.

Align Care Plus aims to deliver premium home healthcare services to your loved ones at affordable rates. Our plans are flexible and can be tailored to your loved one's needs, so you may rest assured that their health is in good shape.

Basic Package

$45 Per Month

Our Basic package includes comprehensive monthly checkups covering weight monitoring, sugar level screening, oxygen saturation, and temperature checks. You’ll also receive a detailed report every month and assistance with medication allowance worth up to $40 annually, ensuring personalised support for your healthcare needs.

Standard Package

$65 Per Month

The ‘Standard Package’ builds upon the Basic tier by including monthly checkups along with medication level checks. It also offers a medication assistance allowance of up to $54 and a free home visit from a GP every 12 months. This tier ensures enhanced medical monitoring and greater peace of mind.

Premium Package

$95 Per Month

Our ‘Premium Package’ offers two checkups every month, all Standard benefits, plus weekly phone calls from our center for added care. Enjoy a medication assistance allowance of up to $84, one free home visit from a GP every six months, and referral support to medical facilities if necessary.

High Flyer Package

$130 Per Month

Our ‘High Flyer’ package delivers top-tier care with two checkups per month and the comprehensive services of all previous tiers. It includes a medication assistance allowance of up to $115 annually, one free home visit from a GP every four months, and seamless referrals to medical facilities for advanced care needs.
